Oftentimes, our new patients are referred to our oral surgery office by their general dentist, while others are not. We do our best to help everyone feel welcome and comfortable. When you arrive, we’ll have you fill out some paperwork and invite you back to talk to Dr. Jacobs. We will take any necessary X-rays or other images, and then you and Dr. Jacobs will create a treatment plan specifically designed with your needs in mind.
Every new patient is required to complete certain forms, but you don’t have to wait until you’re in our oral surgery office to do so.
