Your first Oral Surgery visit
We do our best to help everyone feel welcome and comfortable. When you arrive, we’ll have you fill out some paperwork and invite you back to talk to Dr. Jacobs. We will take any necessary X-rays or other images, and then you and Dr. Jacobs will create a treatment plan specifically designed with your needs in mind. We invite you to explore our office digitally, before your first visit!
Patient Forms
Every new patient is required to complete certain forms, but you don’t have to wait until you’re in our oral surgery office to do so. In fact, we encourage you to download and fill out our forms prior to your appointment. That way, you spend less time in our waiting room, and we can process your information more quickly.
